Intracranial teratoma is an exceedingly rare type of brain tumor, accounting for approximately 0.3 to 0.6% of all intracranial tumors. The majority of these tumors are located in the pineal region. We report a patient with paroxysmal dizziness and headaches. MRI showed multiple abnormal signals in the left temporal and lateral fissure, which presented low signal on T2-FLAIR images and mild enhancement on enhanced scans. The patient underwent surgical treatment, during which we found abundant adipose tissue and hair-like structures in the tumor. Histopathological analysis confirmed that the tumor was a mature teratoma. Primary intracranial teratomas in adults lack specificity in clinical presentation and radiographic features. Variations in tumor location, size, and histologic composition often manifest with distinct clinical symptoms and vastly differing radiographic presentations. Comprehensive analysis, including consideration of the patient's age and biochemical markers such as tumor markers, along with histopathological examination, is necessary for diagnosis.Diagnosing intracranial mature teratomas remains challenging prior to surgery, especially when imaging and endocrine manifestations are atypical. There is also controversy surrounding the treatment strategies for mature teratomas. However, the unquestionable efficacy lies in achieving curative resection whenever possible. Nevertheless, even if postoperative pathological examination confirms a diagnosis of mature teratoma, the possibility of residual tumor tissue containing immature or even malignant components cannot be ruled out. Therefore, despite efforts towards complete resection during surgery, the possibility of tumor recurrence cannot be excluded. While most patients in the literature achieve recovery, they still require long-term follow-up after treatment.
